Co-Curricular Transcript Request
The Keuka College registrar’s office maintains a student’s co-curricular transcript permanently. Requests for transcripts must be made in writing. Phone requests will not be accepted.
Types of Transcripts
Official: signed by the registrar and imprinted with the College seal; sent directly to another education institution, official agency, potential employer or provided to the student in a sealed envelope.
Unofficial: issued to the student for personal use; is not signed by the registrar nor imprinted with the College seal. Note: current students may stop by the registrar’s office or the Center for Experiential Learning to pick up an unofficial copy of their co-curricular transcript.
Holds must be removed from your student account before a co-curricular transcript will be released.
Request your co-curricular transcript
There are several ways to request your co-curricular transcript.
By mail
Complete the co-curricular transcript request form and mail to:

Payment by check or money order payable to Keuka College.
In person
Stop by the registrar’s office, Hegeman Hall, room 221. Forms are available outside the office for after-hour requests. Payment is required at the time of the request. Co-curricular transcripts are not issued “while you wait.”
Payment by check or money order only. Individuals wishing to pay by cash must first stop at the student accounts office to make payment. The receipt issued must be submitted to the registrar’s office with your request.
The first transcript request is free; subsequent requests are $5.
Please allow two to three working days for processing, not including mailing time. However, during particularly busy times such as registration, grading and graduation, processing time may take up to a week.
Current semester activities will be posted to your co-curricular transcript approximately one week after the semester ends.
If you are unable to download the transcript request form, you can write or fax us as above. Please be sure to sign your letter and include the following information:
• full name at the time of your attendance,
• student ID number, Social Security number, or date of birth,
• current address,
• approximate dates of attendance,
• complete address of where to send your transcript, and
• signature.
